Who qualifies & what to bring
- Service area
- Summit or Portage County. Coming from outside that area? Call first. Many still help in an emergency.
- ID
- A photo ID of any kind. It doesn't have to be a driver's license.
- Proof
- They may ask for proof of income.
In their words: “Must be HIV+, receive less than 80% of the median income, and live in either Summit or Portage County. Clients must bring a photo ID. First-time clients must bring verification of HIV+ status and verification of income (benefit letter or 2 months of most recent pay stubs for all in household), OR have a participating agency fax/email a CANAPI Nutritional Program Referral form with attached verification of HIV+ status and income verification.”
