Clients must have an income that follows the guidelines of The Emergency Food Assistance Program (TEFAP) which is approximately 200% Federal Poverty Guidelines. Client may receive service once per month. Documentation Required: Photo I.D. Proof of Address/Residency: Current postmarked utility bill, Ohio Works First (OWF) statement, or other official document such as that received from Social Security. Proof of Income: public assistance benefit letter, Social Security benefit letter, Medicaid Card, or paycheck stub. Proof of Family Size: Medicaid/Medical cards, school enrollment forms for each member of the household, birth certificates.”
